Smilesinc (Smiles Bermuda Ltd) collects and uses your personal information. As you have placed your trust in us when providing your personal information, we want to be totally transparent with you about how it’s handled. “Personal information” is information relating to you as an individual. Smilesinc. will use your personal information in accordance with Bermuda’s Personal Information Protection Act 2016 (“PIPA” or the “Act”), related regulations and guidance notes which regulate the way in which all personal information is held and used in Bermuda.
How do we obtain your information: Your information is either voluntarily provided by yourself when you register as a patient with us or it will be sent to us from another Dental/Medical office. The information is collected by completing our registration form, via phone, email, fax or regular mail.
How we use your personal information: Your personal information is used to treat you as a registered patient. We only collect what is necessary to provide treatment to you.
Who will see or use your personal information: Your personal information will be seen by our Doctors, clinical and administrative staff.
Transfer of your personal information: If you are referred to another dental or medical office overseas we may transfer your personal information to other countries, which may not have the same legal protections for your personal information as those under the PIPA or the UK / EU General Data Protection Regulations. We will ask you to sign a release of documents consent form.
How long we keep your information: We will keep your personal information for as long as we need to in order to fulfil the purposes we have set out in this Privacy Notice. If you cease to be our patient, we will still need to keep some of your personal information to meet our legal and regulatory obligations. We will only keep what is necessary for these purposes.
Your rights You have the right to know what personal information we have, and you are entitled to a copy of that personal data. If we have information that is incorrect, you have the right to correct it and you should inform us of any changes to your personal data.
Sharing your information: You have trusted us with your information, therefore the first thing we want to assure you of is that we do not, and shall not, sell any of your personal information to any third party. However, we may share your personal information with the following categories of companies.
